zVM support dynamic activation of non subchannel set 0 devices

A VM LPAR supports one CSS but it can be 0,1,2,3 but only
supports one SS via HCD - 0. Add support for additional SS

    APAR VM65942 provides support for alternate subchannel sets on z/VM dynamic I/O commands. Here is the APAR description:
    "Alternate subchannel set dynamic I/O support: z/VM 6.4.0 provides dynamic I/O commands within the z/VM hypervisor for
    supporting define, modify, and delete of a device within either the default or alternate subchannel sets. HCD support is currently not
    available."
