Structure CP ESM interface as CP exits instead of building into CP

Implementing a ESM has historically always required rebuilding CP, a daunting task for newbies. Restructuring the CP ESM interface as CP exits would allow adding or disabling an ESM to not require a rebuild or CP IPL.
