Add a COMMENT keyword to all RACF commands

Add a COMMENT (or 'REASON' or 'TRACKING' etc) keyword to all RACF commands and place the keyword text in the SMF record for that command.

Catalog TAKEDUMP command should include VLF asid

Used the "F CATALOG,TAKEDUMP" and "F CATALOG,RESTART" commands to generate the svcdump to diagnose the problem in the pmr. Later told that we should have also dumped the VLF asid so there is insufficient doc to diagnose the problem.
